A day in the life of a MDS Coordinator at Hackensack Meridian Health includes:
Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPís and Triggers.
Maintains and periodically updates written policies and procedures that govern the development, use and implementations of the resident assessment (MDS) and care plan.
Performs administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary.
Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
Participates in surveys (inspections) made by authorized agencies.
Serves on, participates in, and attends various other committees of the facility, as required and as appointed by the Director of Nurses.
Provide written and/or oral reports of the resident assessment/care plan functions as requested or directed by committees or DON.
Ensure that quarterly and annual resident assessments and care plan reviews are made on a timely basis.
Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS. Sign and date the assessment instruction to certify its completion.
Conduct or coordinate the interviewing process of each resident for the residentís assessment.
RN Degree/Diploma from an accredited college, university or approved school of nursing
Must have a Current RN license in the state of NJ
minimum one year experience MDS/ Case Management
